Offre de soins en milieu carcéral en France : infection par le VIH et les hépatites : Enquête Prevacar 2010

Abstract : Introduction – In 2010, health care services in jails related to HIV and hepatitis were studied in the PREVACAR survey, conducted by the Ministry of Health with the scientific collaboration of the French Institute for Public Health Surveillance (InVS). Method – A questionnaire on the provision of health care services was sent to all outpatient consultations (UCSA) existing present in jails in France and the overseas departments. Results – 146 of 168 UCSA (86%) responded to the questionnaire. Almost all of these offers a screening of the 3 HIV, HCV and HBV viruses on arrival but only half renew the proposal in case of denial. A free and anonymous testing is available in about one third of jails. Those serving more than 500 detainees have more frequently hepatology or infectious disease specialists than those of smaller capacity. Chronic active hepatitis C is sought in half the cases of positive serology and accessibility to non-invasive tests of fibrosis differs among institutions. Access to hepatitis B vaccine seems not to be a problem. Conclusion – The strong participation of UCSA in this investigation reveals the strengths and areas for improvement in the provision of care related to HIV and hepatitis in jails.
